How to use .PDF files: |
You will need Adobe Acrobat or Adobe Acrobat Reader, preferably version 6.0.
Right-click your mouse on the link (at left) for the file you want.
Select “Save Target As”
“Save” the file somewhere on your computer (i.e. My Documents)
Open the file from the saved location and fill out the form accordingly by double-clicking in the area where you want to enter data.
When you are finished, you may save it *, print it out, sign it, and mail it (as applicable) to the Chapter 9 Office.

*Adobe Acrobat Reader allows you to enter the fields data online but it will not allow you to save your information. It will only allow you to print out the form. You must use Adobe Acrobat (purchased license) to save your .pdf form work. |
